Pharmaceutical regulator withdraws drug batch after glass fragment found in vial

Poland's pharmaceutical authority has pulled an entire drug batch from circulation after discovering a large glass fragment in one vial. Unable to rule out the defect affecting other packages from the same series, the regulator is removing the product from all pharmacies and wholesalers across the country.
