UK GPs' advice service linked to missed cancer diagnoses, inquiry finds
The Health Services Safety Investigations Body (HSSIB) found that England's advice and guidance (A+G) system — designed for GPs to consult specialists before hospital referrals — is creating risks of missed or delayed cancer diagnoses. The system was intended to reduce hospital burden but has instead introduced patient safety risks across England.
