Two American Airlines jets flew under the same flight number over the US

A Phoenix air traffic controller averted a potential mid-air collision after noticing two American Airlines aircraft were mistakenly operating under the same flight number. The controller acted quickly to separate the planes. The cause of the duplicate flight-number error is still being investigated.
