Enterprises Deploy AI Agents Without Oversight Despite Widespread Production Failures

A June 2026 VB Pulse survey of 157 enterprise respondents found that half of companies have deployed an AI agent or LLM feature that passed internal tests but still caused a customer-facing failure, with one in four experiencing this more than once. Despite this, 66% of enterprises already allow some production deployments without human review, or plan to do so within 12 months. Only 5% fully trust the automated evaluation systems driving those release decisions — a mismatch researchers call the "evaluation gap."
