PPK pension scheme: auto-enrolled after 3 months — how to opt out?

Poland's Employee Capital Plans (PPK) automatically enrol most employees on a standard employment contract after at least three months of work. Participation is not mandatory — to opt out, an employee must submit a written declaration of resignation to their employer.
